Name:
Heather Kuwalik-Drake
Title of the Degree I am Seeking:
(eg. MED: Teaching and Learning: Student Services or MED: Program Specialist: Math Specialist or MAT: Special Education)
Master of Education: Curriculum & Instruction in Special Education (Non-Licensure)
Current Position:
(eg. elementary teacher, principal, etc.)
Special Education Instructional Assistant
City and State Where I Live:

Directions: Use the “Response” section to address the prompts below. (Be thorough in your responses. A nicely written paragraph with at least 3 to 7 sentences per prompt is expected. Be sure to use complete, descriptive sentences that fully explain what you are trying to convey. The more description you provide, the easier it will be for your instructor to understand what your project is going to be and why it is important.)
1. Project Description: What is the actual project or product that you will be creating /
producing? Now, describe this project in detail.
Response: The actual project or product that I will be creating/producing is a substitute folder. The
substitute folder will contain all of the necessary information, documents, and resources
necessary to maintain classroom operations. Being a substitute within any classroom can be
overwhelming; however, being a substitute in a profound mentally disabled (PMD) classroom is
another story. For students with autism spectrum disorder, it is imperative that there be as few
interruptions and changes as possible. Creating and implementing a substitute folder will help to
ensure that the substitute as well as students are able to maintain a sense of routine and support. Page 1 of 4

2. Rationale: State why you chose this project topic. How does this project fit into the broad area of education? How does it relate and/or benefit to your current work position or the job you are seeking?
Response: I chose this project topic because it is important to provide substitutes with the necessary information to promote effective collaboration as well as ensure students remain successful academically. As a special education instructional assistant, I have experienced first-hand how a lack of information can impact the educational environment as well as the students. When substitutes feel overwhelmed or under-prepared, the students are impacted the most. 3. Statement of Purpose: State what you plan to accomplish with your project.
Response: I plan to accomplish my project by creating a Microsoft Word document that will be divided into
different sections to ensure that the substitute will have all of the necessary information to maintain classroom operations in a safe and supportive manner. The substitute folder will include:
•
Class roster •
Pertinent student information (i.e. accommodations, allergies, special area schedule)
•
Seating chart
•
Classroom staff information
•
Contact information for school personnel •
Schedules
•
Map of the building
•
Emergency procedures
