I am the physical therapy team leader at a pediatric outpatient physical and occupational therapy clinic. This clinic evaluates and treats children between the ages of birth and 21 years with a variety of conditions, including, but not limited to: developmental delays, orthopedic and neurological conditions, brachial plexus injuries, torticollis and plagiocephaly, and autism spectrum disorders. Occasionally, the therapists will evaluate and treat an adult patient with developmental disabilities. The clinic is open five days per week with four occupational therapists, three physical therapists, one physical therapist assistant, and a Spanish-speaking interpreter. As the physical therapy team leader, I work with the occupational therapy team leader to suggest, establish, and implement new programs at the clinic, under the direction of the therapy manager.

It was difficult to find the regulations for physical therapy assistants because the information was interspersed in the statutes and rules and in the administrative Code. Much of the law was written more for physical therapist guidance than directed towards the PTA. To apply for an assistant license, the person must be of good moral character, a graduate of an accredited PTA school, pass a board exam, and jurisprudence exam. The license is a certificate and no continuing education is required at this time. The PTA can only practice under the direction of a physical therapist. The assistant cannot evaluate, re-evaluate, discharge, or change a program of a patient and the physical therapist responsible for the patient must be onsite during each treatment. The exception to this rule is in a general supervision capacity. The physical therapist assistant with 2000 hours of supervision in an onsite facility can see a patient independently without onsite supervision (Azsos.gov, n.d.). The physical therapist must be able to communicate with the PTA within 15 minutes, re-evaluate the patient every 4th visit or 30 days whichever comes first and visit the patient the day of communication if needed. I have previously worked with physical therapists while I was a behavioral therapist at an autism clinic. Some of my patients required a multi-dimensional approach with many specialties including PT, OT, and SLP. Prior to this experience I assumed physical therapists had a very limited role in the health care setting. I thought their only focus was on the elderly and post-surgical patients, but then I realized it is a much broader specialty. Physical therapists can also specialize in neurological disorders, special needs children, and many other aspects of the patient’s overall health.

Pediatric physical therapy was one of the practice settings that I wanted to become further educated in. This being said, I was not very familiar with this setting prior to researching this topic. After reading, something that I found appealing about the pediatric setting is that it is a collaborate effort between physical therapist, child, and family. I also found it intriguing that pediatric physical therapists assist children throughout age-related movement stages as they develop. The thing that I thought would be difficult at times is communicating with parents. My first three weeks of clinical were my roughest, there was so much information being thrown at me. It didn’t help that I was working in two different school settings: District 75 (more severe diagnosis and most restrictive)/ Regional (general education with children that had least severe diagnoses). Understanding, what the DOE is, the do’s and don’ts of a school based physical therapist and much more were areas of information that were minimally taught during the curriculum. Trying to learn and understand as much information as possible lend me to think, that the school based therapist is almost a whole new field. The school based therapist used tests and measures, interventions, and created goals and treatment plans much different from any other setting I had been in previously. This was a challenge, a challenge I was willing to understand and conquer.
