I have spoken with you and MJ about the lack of support administratively in the past. There are items that I’ve reached out to MJ about, requesting assistance and either never get a response, or get a dramatically delayed response after multiple follow up attempts. I have listed a few recent examples below:
FAS - I know on the call you mentioned that the FAS responsibility doesn’t have to delegated to me, however, in December 2016 you made a direct request for me to take over the FAS responsibility as of January 1, 2017. His account was left current, so on 12/8/2017, I sent an email reminding her about the skip. She failed to move this resident out and/or start the paperwork. On 12/11/2017, I reached out to a resource for advice about the skip process and was informed the resident would need to be moved out immediately, so on 12/11/2017, I moved the resident out and sent MJ an email confirming that I moved him out of Onesite and offered to complete the FAS.
Bradford’s unauthorized pet – In February 2017, our maintenance conducted preventative maintenance in 2500 building. They noted that there were two cats in the apartment with only one registered. We contacted and charged each resident with unauthorized pets, besides the Bradford’s because Marie spoke to them and they said they got rid of the unauthorized pet. This was false as maintenance went into their apartment on 12/11/2017 for a few service requests and confirmed there were two cats and carpet damaged caused by their pets. I sent a detailed email to MJ on 12/11/2017, with no reply. I verbally followed up and MJ said she doesn’t want to contact them because she knows “they’re going to start drama”. Their account has not being charged as of 12/27/2017.
Most times, I don’t even reach out for assistance unless it’s something I absolutely could not do without MJ assistance as it is very frustrating to ask for help due to the lack of support. Kharin, Becca,
SC received a telephone call on 10/16/2015 stared 9:34 and end at 9:41 am from Tricia Crooks at Liberty Resources Home Choices (LRHC) Community Outreach and Enrollment Leader. Stating that she spoke Pa and he wants to resume his service order with LRHC for PAS service. SC informed SC that this information will first need to verify with Pa. SC expressed concerns about LRHC being able to fulfill service since they had the case unstaffed for over two weeks (09/25/15-10/15/2015). Tricia apologized on behalf of LRHC, and stated that they have someone assigned and is ready to go all is needed is the resumed service order ASAP. SC again explained to Tricia that Pa has to confirm this besides Pa was very adamant about switching provider because the
Medical: Ms. Williams failed to submit her medical documentation. Ms. Williams is expected to submit her medical evaluation or physical by 03/2017. Case Manager explained to Ms. Williams that if she failed to submit the documents required she would be considered as non-compliance.
This officer has had contact via face to face and phone with the mother, Ms. Evelyn Edwards. Discussions have taken place with her regarding Marquel’s Comprehensive Re-Entry Case Plan. She has participated in a video conference with Marquel since his commitment. She is aware of his placement in the Virginia Beach CPP Program. She has not visited him at the facility, but she has had contact with him via phone. She is currently unemployed. Upon Marquel’s release from DJJ, the anticipated parole plan (CRCP) will be for him to return to her home with intensive supportive services via 294 funding. My name, Jolon Rietveld I am currently living with my dad. I have been supporting myself in Saskatchewan for 2 years at a private school. This school only had 80 students in total so I had lots of 1 on 1 time with my teachers. So I will be struggling for the first little bit until I get use to the big high school life. My mom is living in Alberta. Her career choice is long road truck driver; she was an Ontario swat team in waterloo reign. I have been supporting myself since grade 9 and started paying for everything myself besides food since grade 10. My mom does love me, but she just has a different way of teaching life skills than other parents.

2/2/16 CM completed the intake with Elsie Boone, Brian Reid/Son/POA, Patrice Reid/Daughter In-Law, and Gladys Burnett/Aunt. Ms. Burnett provided Mr. and Mrs. Reid with Elsie bills (balance on rent, water, cable). Ms. Burnett announced that as of today she will no longer be the payee for Elsie and she relinquished all of her responsibilities. During the intake Ms. Burnett attempted to instigate past situations involving the parties. The Reid’s did not address the conversations and asked CM to continue with the intake. Due to the adult learning classes and Elsie's determination she did better on the LOF and therefore still did not qualify for the ID Waiver.
In your case, since he is your direct report, we can use your letter as the supervisor letter; however, if you have someone else in the reporting line (above you) that would like to provide a letter of support as a supervisor for both you and the nominate, that would be acceptable. The goal would be to show as much support for the nominee as possible form other members/Fellows. Either way is acceptable in your case.
