Community Teaching Work Plan This is a Community Teaching Plan Work Proposal group project for the city of San Diego, and our target community is children in elementary school. We have chosen primary health promotion/prevention and will address Childhood Obesity, which falls under the category of Nutrition and Weight Status in Healthy People 2020. We developed four behavioral goals for out proposed teaching plan:
1. Kids will learn the importance of incorporating 5 servings of fruits and veggies daily; they will learn about the significance of “rainbow foods”
1.The richly pigmented foods in these color groups: Red, Yellow/Orange, White Green, and Blue/purple have nutrients that are not found anywhere else. 1. We will provide coloring sheets for the younger children with several fruits and vegetables in each color group. We will provide an attractive display of rainbow foods for sampling.
2. Parents/students will verbalize three healthy items to consume using the food pyramid.

Consuming a variety of nutrient-dense foods within and across the food groups, especially whole grains, fruits, vegetables, low-fat or fat-free milk or milk products, and lean meats and other protein sources. Limit the intake of saturated and trans fats, cholesterol, added sugars, sodium (salt) and limit caloric intake to meet caloric needs (Healthy People, 2012). 2. During this portion of the assignment the parent/student will create a healthy meal with snacks using the food pyramid, giving one or more example why it is a healthy choice or not a healthy choice.
3. Promote physical activities
identify the educational preparation and role(s) of the clinical nurse leader (CNL) designation. Give an example of how the CNL influences direct patient care whether in a hospital or out in the community.
Close contact with anyone who is experiencing a fever, cough or shortness of breath and has traveled from the Arabian Peninsula within the past fourteen days should monitor their health beginning with last day of exposure and seek medical attention if they become ill (U.S. Department of Health and Human Services, 2015).
In early April 2013 a measles outbreak was discovered in North Carolina. By mid-May the outbreak had been identified in Stokes and Orange Counties via 23 active cases. Every case was linked back to a family that had spent 3 months in India and had not been vaccinated. By the 16th of April the state laboratory of Public Health was able to confirm the diagnosis, with the last known case being confirmed on May 7th. The investigation of this outbreak revealed 4 patients with a confirmed diagnosis that had received one of vaccination of the two part series. The other 19 cases had not ever been vaccinated.
There also needs to be several options each day, so that kids have a choice.. The foods need to follow the “food pyramid” in order to provide a balanced daily intake. The largest amount of food that needs to be consumed are vegetables, salads and fruit. The next level are foods like cereal, breads, potatoes, pasta and rice. Then, the next should be the dairy category (milk, cheese, yogurt), proteins (meat, eggs, beans, and nuts), and fats (spreads and oils).
Childhood obesity has expanded tremendously within the past thirty years (CDC, 2015). It is not only a state, but also a nationwide issue. For many children, they depend on their school lunches to provide them the nutritious meals they cannot afford to have at home. As a community, we need to get our children into better shape. Not only will they become more astute, but they will also live healthier lifestyles, and have less health complications as they age. When you are overweight or obese, you are much more likely to develop health problems like heart disease, diabetes, or even a stroke. It is our responsibility as a community, state, and nation to offer nutritious meals and activities for our youth and future.
The fundamental standards of the eating regimen are that the individual can eat a wide range of beans, vegetables, organic products, grains, and vegetables, however just 10% of the individual's calories ought to originate from fat. The individual ought to keep away from meat (red and white), oils and items containing oil including avocados, olives, nuts, seed, full-fat dairy, and sugar. It may not be perfect, and given they are kids it won't be, but if they choose a banana over a candy bar just a few times a week it will make a huge, long-term

Curriculum revisions that require parent or guardian involvement would be conducted outside of the classroom; and be labeled “health field days”. These “health field days” would incorporate knowledge about healthy nutrition and physical activity, while performing physical activities. Parent participation would be required and/or encouraged to help educate parents on the risks and consequences of obesity, as well as additional healthy nutrition habits and practices. Physical activities would be designed to be culturally sensitive and understanding. A development of a school vegetable garden may also be developed. Developing a vegetable garden would encourage physical activity as well as provide additional healthy food education.
According to the Centers for Disease Control and Prevention, between 2011 and 2012, 17.7% of six to eleven year olds and 20.5% twelve to nineteen year were considered obese (Centers for Disease Control and Prevention). That is a very disturbing number of children and young adults that are obese. It has also been found that obesity is higher in the states of Tennessee, Georgia, and Alabama. From data collected in Hamilton County found that more than 35 percent of school aged children are at high risk of becoming overweight (Belz). Also, according to the Pediatric Healthcare Improvement Coalition, Tennessee is the 8th worst state for Child Obesity. One of every three children are overweight, and two of every five obese adolescents become severely obese adults (Coffman). Since Tennessee is ranked as the 6th most overweight state in the country, all schools should be more aware of what they are feeding their
Diet- It is important to maintain a balanced diet consisting of 50-60 percent of carbohydrates (avoiding refined foods) about 20 percent fat nad rest as protein.
The program that I did was having different interventions that tackles health issue of obesity. In my interventions there are physical activities, nutrition, and knowledge about obesity that can help reduce or eliminate threats of obesity. My program’s goal aims to lower the risk of obesity and giving support. My interventions will be cooperated with community in Oakland and having schools, family, and the community to coming together for this event. There will be booths and stations that teaches the perspective of healthier choices among elementary students. Elementary students are the main target for the program to enhance their learning and knowledge about obesity. In Oakland, there is a high rate of obesity among elementary students
During the past weeks, in health class, we have been learning about diet and what makes up a healthy diet where there is no danger of you getting some sort of sickness or health problem. In the process we had decided that what made a plate healthy was balance. You had to have a balance between macronutrients and micronutrients with what you ate and so we learned about the healthy eating plate. This plate gave us an idea of what a meal of the day should look like. It should have 25% protein, 25% whole grains, 30-40% vegetables, and 10-20% fruits. We discussed why it was accurate and we said that there should be a balance, it shouldn't be like eating the healthy eating plate in every meal but eating it throughout the day. So to experiment and see what eating healthier was like, we tried to eat the healthy eating plate throughout an entire week. And let me tell you that it was not easy, in my opinion.
Limit foods that are high in fat and processed sugars, such as fried and sweet foods.
