The most significant is the use of power in all three-system levels. The comprehensive learning on how nurses, leaders, organizations, and various government agencies utilized their power, and the input from other students have revealed to me the benefit of legitimate use of power, and the consequences when power is illegitimately utilized (Ottenberg, et al., 2011). When addressing the power utilization in all the health system levels, the most urgent issue that came to my mind is the advanced practice nurses inability to again full autonomy to practice to their full potentials in some states, despite the institute of medicine (IOM) recommendations. Since the capstone project started early on beginning of the Doctor of Nursing Practice (DNP) program, I was able to discuss and work personally with Dr. Spicer as my practice mentor in selecting the appropriate topic for my project. Dr. Spicer was very involved in choosing my second option for new capstone project # 1 related to simulation lab and faculty teaching and learning. The patient simulation laboratory (SIM lab) uses state-of-the-art equipment that provides students and faculty with the opportunity to tackle real-life scenarios in a safe and supported environment. Further, I learned that active learning involves the faculty through participation and investment in exploring content knowledge in all phases of the learning process. It requires educational activities that provide faculty with the opportunity to engage actively in courses and respond to the learning situation.
“The purpose of public policy is to direct problems to government and secure government’s response, while politics is the use of influence to direct the responses toward goals” (Milstead, 2013, p. 12). Policies are used as guidelines to reach a common goal. In healthcare, policies can affect and have an impact on how we provide care for our patients’. For this reason, it is important for nurses to be knowledgeable and involved in important healthcare decisions. “Knowledgeable nurses in advanced practice must demonstrate their commitment to action by being a part of relevant decisions that will ensure the delivery of quality health care by appropriate providers in a cost-effective manner” (Milstead, 2013, p. 1).
Healthcare in the United States is facing numerous issues within the current and future turbulent times. I believe that two major issues are the disparity of provision of healthcare to varying societal groups as well as the projected shortfall of healthcare providers to adequately service the population in general.
As resistant as some states’ legislative and regulatory bodies are to grant APNs autonomy of practice, the damage being done by over-regulation is clear (Safriet, 1992). Physicians are forced into a position to either supervise the APN’s practice or be constantly consulted for approval of their practice decisions. Safriet (1992) described that in and of itself, this constant supervision may appear to patients that the APN is not competent to provide adequate or care equivalent to that of a physician. If the role of the APN is to bridge gaps in health care by relieving the medical establishment of some of the patient load by performing the same function as a physician in a primary care setting, it seems wholly unnecessary to restrain their scope of practice in those areas. This type of restrictions affect cost and patient care accessibility (Safriet, 1992). This was a problem stated in the article, however 25 years later, populations of patients remain unseen or cared for and APNs continue to be underutilized (Safriet, 1992). Rigolosi and Salmond (2014) cite the American Association of Nurse Practitioners (AANP) when they state that not utilizing nurse practitioners due to practice restrictions costs $9 billion annually in the US (p. 649).
The Carnegie Foundation reports that nurses today “are undereducated for the demands of practice” (Benner, Sutphen, Leonard, & Day, 2009, ¶ 17). Education of the ADN centers around the hands-on skills needed to care for a patient at bedside and less on the scientific theories that allow the holistic treatment of a patient.
