Introduction:
I have taken Ten concepts for this conceptual learning project from the modules we have studied so far and see how it will applied to a context related to my life, social context , work place , my community and world around me.

1- Health, wellness and illness Health: “Health is not a condition; it is an adjustment. It is not a state but a process. The process adapts the individual not only to our physical but also our social environments.”This definition emphasizes health as an adaptive process rather than a state.
Wellness: Wellness is a state of well-being. It means engaging in attitudes and behaviors that enhance quality of life and maximize personal potential.

The roles we play vary amongst social situations and the people we interact with. The person we are in a professional setting differs from the person we are in a casual setting. I decided to shed some light on the diverse roles of nurses while in the workplace. Since I work at a hospital here in town, I have the opportunity to see these nurses in a professional setting and a casual setting. It’s always interesting to see the dramatic difference in behaviors while being in a patient’s room versus being in the breakroom.
Wellness is the health of a person as a whole. It encompasses the person’s physical, as well as mental and emotional health. Over time, therapist came to the realization that clients needed to be well in every aspect of their lives in order to be healthy ("Wellness in Mental

By looking at what different people say about health, it is clear that each one look at it differently. Therefore, there are many ways to define health. When we look at health, all physical, intellectual, emotional and social aspects should be considered because they all affect our health equally. Health can be defined in many ways, positively, negatively and holistically. It depends on how the individuals look at their health and how concerned they are about it. Our state of healthiness will depend on many things such as where do we live, what job do we do, what do we eat, who are our friends and everything else related to our lifestyle. Our choice about our lifestyle can make a difference to our health. If we are interested enough in ourselves, we can improve our own health. Some people think, that it depends only on the health care we obtain; however, it doesn't only depend on this. It is connected to our lifestyle as well.
Stereotyping is when a judgement is made on a group of people because of the actions of a few. Stereotyping happens because of misinformation and other people’s opinions.

This report is about Harry a 66 year old male service user who was diagnosed with Chronic Obstructive Pulmonary Disease (COPD) since he was 55 years old. He was a very heavy smoker during his mid 40s which led to his weight gain. And this also led to his breathing problems. Two years ago Harry had a stroke which left him loosing his physical functioning abilities on his right side. After his stroke Harry was moved into a nursing home.
For this discussion board post I will be discussing the healthcare delivery system in the community in which I live. I will list the name and address of the system, as well as, the mission of the health department. Then I will give the history of the health department. Following this, I will discuss the health commissioner as well as the services that are provided by the health department. Next, I will describe the role of the public health nurse and then the department’s emergency planning components. Then, I will state the services that are not provided that I think should be.
The population health needs and community health resources are to identify the health needs of the defined population, available health resources in the community should be considered and opportunities for collaboration established. Gaps in health services can be identified by comparing identified health needs to existing health resources. The wide range of skills and expertise of interdisciplinary providers in Family Health Teams need to address many of the gaps in health services. Services being delivered are to patients must be fully accessed and practiced before the service is being provided. These services will be delivered with internal and external providers including Community Care Access Centers, Public Health Units, Independent Health Facilities, and hospitals will be required to coordinate services and avoid gaps or duplications of services. All in all, the government’s plan should be realistic, measurable, and meaningful and focused on outcomes such as impact, quality, patient satisfaction, timeliness, and efficiency. Currently in most US healthcare facilities system theory is not used. Many practitioners seeing the same patient do not share information or share ideas resulting in a system that has high errors and low quality of care. As mentioned above hospitals that have adopted systems theory have increased quality of care and reduced errors.
The Trinidad health care system operates under two tier system which consist of both public and private health care facilities, the main responsibility of the ministry of health is the overseeing of the health care sector, although this agency does not directly run the public health care facilities, the responsibility of this office is to set policies, goal and targets areas based on the assessment of the community health needs, as well as to allocate funds to the regional health authorities to finance the various health needs of the community.
Healthy Village Action Zone program has been operating for nearly three years in partnership with Auckland District Health Board (ADHB) and three other primary health organizations (PHOs) whereas, the District Health Board (DHB) had provided with funds for the churches to purchase resources for the particular health programs. “Survey conducted in 2007/2008 showed that 16 percent of the around 120,000 inpatients treated at ADHB that year were Pacific” (Manchester, 2010). Throughout the Healthy Village Action Zone, the most affected inpatients are Pacific people with high rates if pediatric rheumatic fever, diabetes, stroke and cardiac heart disease.
