The team leader is usually an RN (King, 2013). Describe Nursing Care Delivery Model: a) What personnel are utilized? In team nursing the care of the patient divided between team members- licensed and unlicensed personnel whose work is coordinated by the team leader, most of the times- RN appointed by the charge nurse. b) What are their roles/responsibilities? In team nursing the responsibilities usually divided in between the personnel, so that everyone’s has its own role in patient’s care. For example, RN might be assigned for patients’ assessments, LPN assigned for certain medication administration and nursing assistant helps with care/bathing and feeding patients. All this done under supervision of a team leader another RN (Currentnursing.com, 2015). c) How is the work coordinated? The appointed team leader is coordinating the flow of the work done. The team leader gives directions and instructions in details to team members, he/she is also schedules the care and assigns the tasks in between the team members. d) What are the reporting relationship? Teamwork and collaboration in nursing can be defined as the development of partnerships to achieve best possible outcomes that reflect the particular needs of the patient, family, or community, requiring an understanding of what others have to offer. (Giddens & Liesveld, 2013). This means that nurses are constantly working as a team to provide the best care for their patients. Working as part of a team may include listening to the client, getting help from another nurse to verify dosage calculations or working alongside the doctor to carefully facilitate their orders. I don’t believe any one nurse would be effective if they were working by themselves. The profession alone is demanding and one nurse working by themselves to assist the needs of more than one patient at a time would be dangerous.
Care delivery models are an integral component for delivering patient care. With the collaboration of other members of the healthcare team, the Registered nurse is able to fully optimize his/her skill sets to provide to best quality care. As discussed consistency and coordinated care are the key. Studies must be conducted and evidence base practice must be implemented in order to find the model that is suitable for a particular unit. To be able to perform those tasks a care plan must be made for each client. An RN manages the health care services such as observing and assessing the health status of clients and implementing/assessing nursing care. This all falls under the initial assessment of a client, which is within the scope of an RN. An RN uses information gathered as part of client assessment, they then have the capacity to assign client care to other members of the nursing team, RNs and LPNs, and assign tasks to other care providers such as nurse’s assistant. In addition, a well-functioning team is led by a good leader; a leader does not necessary mean the CEO, but someone who is driven, knowledgeable, and a good communicator. During the team formation, a number of challenges manifested. Firstly, it was the fact that there were nurses from other units. Hence, there were fears that they may not be well informed with procedures undertaken in our unit. During my own personal experience in nursing, I have had the opportunity to witness teamwork at its best, and shamefully, teamwork at its worst. During a shift on a neuro critical care floor, I was assisting a nurse turn and change a patient. Nearly every time we shifted the patient, an alarm would sound. Within moments there was a voice on the other side of the curtain asking if help was needed. Next time the alarm sounded; there was the helpful voice again. It never failed. How comforting for the nurse this must feel, and ultimately, better outcomes, as well as safety for the patient. At the other end of the spectrum, I have watched as a nurse asks for help, and is turned down multiple times. I have heard the awful phrase, “that’s not my job.” I have seen patients code, and the nurse spends the next hour caring for that patient including resuscitation, and transferring to an intensive care unit. No one takes the initiative to check on the nurses patients while she is gone, not even the charge nurse. Clearly this is not a positive or safe work environment for anyone. Teamwork
Urgency of acute care varies depending on the situation but can range to anything from emergency surgeries, to injuries, chronic illnesses, and also for the recovery of those procedures. Majority of the patients in acute care settings are critically ill. Nursing responsibilities in acute care settings are vital to patient’s recovery due to the front line position nurses play as well as the wide variety of tasks carried out. Assessments are made during every encounter the nurse has with the patient along with monitoring the patient’s progress. Nurses are responsible for recognizing symptoms the patient may be experiencing due to illness or injury and whether they fall in the spectrum of normal reactions. Vital signs are measured routinely and can be indicators of the patient’s current status. When vitals are questioned diagnostic tests can be arranged to further assess possible comorbidities the patient may have. Care plans are made to plan interventions the health care team can take to help patients through challenges they face, both physical and mental. Nurses administer medications as well as first aid as needed. They are responsible for maintaining special equipment patients may require including monitors and ventilators are well.
