Throughout my time on the Mother Baby Care Unit, I have faced many instances in which I have been able to reflect on both my patients and the care that I was providing them. One situation in particular that I found myself critically reflecting upon involved a new mother who was feeling slightly stressed about being discharged as her newborn son would not be going home with her. For confidentiality reasons, this patient will be given the pseudonym of Kayla for the remainder of this reflection. Kayla and I met on the morning of her discharge while she was finishing packing up her belongings. At this time, I completed her final set of vital signs and began to talk with her about going home. As I spoke about this topic, I noticed that Kayla was becoming slightly stressed as her voice sounded a little shaky and she was increasingly restless. I then proceeded to interpret why she could be feeling this way to which I realized that as Kayla has yet to be more than one hospital zone away from her son and the Neonatal Intensive Care Unit (NICU), she was likely feeling quite uneasy about going home. Like any mother with a sick child, she did not want to leave her son’s side during this tough time and by being discharged, Kayla may have felt that a part of her would no longer be with him. In today’s world, where globalization and the internet has shrunken spaces and reduced distances, it is only natural that we come across people of different cultures living in harmony. As a nurse practitioner, one experiences several circumstances where the patients are from different cultural backgrounds, varied religious beliefs and also from the other part of the world. While a nursing practitioner has a challenge of treating every patient with utmost care and concern, there are times where our strengths and beliefs come in between our duty (Maier-Lorentz, 2008). A nurse practitioner might have to face situations where they might not have any control over, and still should show their best face. This is where their values, beliefs and behaviors might help them to achieve the impossible.
Critical reflection is vital to develop evidence based practice for safe and quality approaches to professional nursing practice. Nursing professionals should critically reflect on events to identify what health professionals might do to improve their practice and reduce the risk of a similar error. Reflective practice can help to learn from their mistakes, be empowered and most importantly to deliver best possible care to patient as nurses must work closely with their patients to develop a therapeutic relationship. Twenty-one years ago, my parents received the devastating news that my lungs were not developing properly and as a result my mom had an induced labor. I was born as a premature baby, weighing only 3 lbs. and was placed in the care of neonatal nurses in the Neonatal Intensive Care Unit (NICU) for 18 days. While my mom came to visit me, she would notice how well the nurses would care for me. Though their jobs were tough, the nurses would often carry me while I was crying and would often talk me to sleep, sometimes singing lullabies.
Reflection is a process of exploring and examining ourselves, our perspectives, attributes, experiences and actions / interactions. It helps us gain insight and see how to move forward (Nursing Times 2018). I believe reflection is particularly important when it comes to Nursing, as medicine is constantly changing/ improving and us ourselves medical professionals must adapt with the changes in medicine. I find that reflection is extremely useful in doing this as we can look over procedures or experiences that we have had, how that made us feel, whether we would change anything, then in the future we can see the changes that may have been made, whether this has changed our feelings and opinions on medical practice.
Skin integrity is an important concept that’s nurses assess on their patients. A key skill in nursing practice is to frequently assess the skin for possible breakdown or decreased skin integrity. Skin assessments should be conducted thoroughly once a shift and frequently reassessed for any signs of change. Skin discrepancies may be the first sign of an underlying issue. Early detection of any breakdown can help to implement interventions sooner. Unfortunately, unless there is a major skin discrepancy, skin issues can easily get overlooked, specifically in documentation and report. The focus of this paper is to research new skin integrity assessments to improve documentation effect and accuracy, resulting in decreased prevalence of skin breakdown in hospitalized patients. Topics discussed include reviewing current practices and new skin assessment techniques that decrease the prevalence of skin breakdown and pressure ulcers.
This essay will discuss a clinical skill in which I have become competent in practicing as a student nurse.
She told the nurse that she was not enjoying her pregnancy but believed that she will learn to love her baby once it is born. She went on to state that she had not discussed her feelings with her husband and family and asked the nurse to keep the conversation private.
This objective will affect my future nursing practice because conducting physical head to toe assessments and obtaining vital signs is a different process when working with paediatric patients rather than adults. It is crucial to note any changes that I can catch throughout my assessments, and vital signs since paediatric patients might not be able to communicate with me if they feel any changes in their health. By understanding the anatomy and physiology, the developmental stages of children, and obtaining accurate vital signs and assessments, it will increase my knowledge, skill, ad judgment to successfully complete and distinguish any abnormities and changes during my assessments. Also, knowing the developmental stages and distraction therapy method techniques can help me provide thorough examinations and find ways to help my patients cope if they are feeling anxious and scared.
Nursing can be a demanding career, but the benefits far much outweigh the challenges. Most importantly, it’s the rewards it offers by allowing an opportunity to make a difference in another person’s life through the provision of care when they need it. Just as Patricia Benner theorized in her book “Novice to Expert,” nursing encompasses both educational knowledge and extensive clinical experience acquired throughout one’s career. This far, I continue to acquire knowledge and clinical knowhow which will promote proper and efficient care to patients. Since I began practicing one year ago in a long-term healthcare facility, I have interacted with patients, families, physicians and other members of the healthcare team to coordinate patient’s care which has enabled me to gain confidence in myself. While I cannot deny that it was difficult to transition from a student to a licensed nurse, I learnt to overcome these challenges and focus on my strengths. Practicing as an LPN has provided a platform to learn and gain experience even though the duties and responsibilities are limited by the scope of practice.
The following essay is a reflective account on an event that I, a student nurse encountered whilst on my second clinical placement in my first year of study. The event took place in a Fountain Nursing Home in Granite City. The Dreams and the Dilemmas is a unique video that highlights the real life work and accomplishments of nurses, physicians, and social workers performing extraordinary acts of caring and humanity towards the couples and their premature twin sons. After watching this video, I released the importance of teamwork and good communication in the Neonatal Intensive Care Unit to the family. The physicians gave the couples options to choose from, even though they knew Travis was not going to make it with any of the opinions giving. And when the couples chose not to take off the life support, the physician respected that decision. He said in the video, “we will continue to do everything we doing to Travis”. They made
It was nurses that got us through that time with our sanity intact. Nurses checked on my mother—and us—multiple times an hour. They ran tests, updated charts, and changed IVs; they made us laugh, allayed our concerns, and thought about our comfort. The doctors came in every now and then, but the calm dedication of the nurses was what kept us together.
She told me that she and the patient’s father had been trying to convince him to take the meds, as they both believed it would be best for him, however, he was not yet convinced. She told me he had recently started doing his own research online, which seemed to be improving his outlook on therapy. I encouraged their efforts and expressed my thankfulness in his self-directed research, as I recognized this as a sign that he had entered the contemplation stage of the transtheoretical model (Hall & Edgecombe, 2014, p. 297), which showed progress. During this conversation, I noticed that the mother was showing concern towards her son, and appeared distressed by the situation. I asked her about her experience as a psychiatric nurse, and she told me that despite her decades of experience, it has been very difficult for her to watch her son struggle, as it is completely different when it is “so close to home”. While validating her challenges and offering supportive comments, I encouraged her to see her nursing knowledge and expertise as a strength to her son’s recovery, as well as her own well-being, through this difficult time. Starting the Registered Nurse (RN) to Bachelorette of Science in Nursing (BSN) program at Jacksonville University in June 2015 was a simple transition for me. I had graduated an Associates Degree in Nursing (ADN) program in December 2014 and became a licensed RN in January 2015. I had just gotten hired for my first nursing position the month prior to the start of class and had not yet started working. I was very much still in student mode. While this makes it a bit easier to learn and process information in a classroom setting, I had no real world experience to help guide me. Through the knowledge I have gained as a student at Jacksonville University I have become a competent and well-rounded nurse. I am able to quickly problem solve and utilize critical thinking to identify and rectify issues. I have even put together, and will be presenting, and presentation about nursing practice for staff members at the facility where I work.
This essay is a discussion about my experience during the first two weeks of my clinical placement in an older adult ward. My experiences will be demonstrated using the various nursing skills acquired thus far.
