Mary, a 79 year old patient has gotten up from bed without assistance to use the restroom. She falls on the way and hits her head leaving her with a huge laceration and possible subdural damage. This is the third fall this month on the unit; changes must be made in order to address the policies to prevent falls. Policy changes are one of the many issues that a quality improvement nurse will address in her field. Nursing is a growing and evolving field that changes with new initiatives, government regulations, and technology. There is great demand in the field and new nurses are constantly called to meet the needs of patients. As the population of our country ages, the number of nurses needed to meet these demands will increase. The field of nursing does not just involve the direct care of patients. A number of careers within the field support policy, management, and coordination of patient care. Quality Improvement nursing is one career that can be obtained by a registered nurse that does not involve direct patient care. Quality Improvement nursing involves initiating policies and changes within the facility to improve patient care as well as lower facility costs, increase patient safety, and increase nurse satisfaction. This career has grown drastically in the last few decades due to health care reform acts, insurance regulation changes, and advances in technology. The position of quality improvement requires a unique set of skills and training, a higher salary matrix, as
care outcomes, as well as benchmark data to assess current practice,” (Sherwood, 2014). American Nurses Association has a National Database of Nursing Quality Indicators looks for a cause for common healthcare issues, such as “surgical site infections, pain assessment, pressure ulcer development, and falls,” (Sherwood, 2014) to better understand how these things came about to possibly prevent in the future.
In the article “What Do Nurses Really Do?”, Suzanne Gordon explores what nurses truly do. She concludes that nurses “save lives, prevent complications, prevent suffering, and save money” (Gordon 2006). Nurses provide care for their patients in the physical and emotional sense. Emotionally caring for a patient and being sensitive to his or her needs result from interacting with patients while performing the skills and using the knowledge that nurses learned in school. Nurses grow in their skills, knowledge, and attitudes through practice. Quality and safety education for nursing incorporates competencies that all nurses must use in their practice. These nursing competencies include evidence-based nursing practice, quality improvement, safety, teamwork and collaboration, patient-centered care, and informatics.
The overall goal through all phases of The Quality and Safety Education for Nurses (QSEN) is to address the challenge of preparing future nurses with the knowledge, skills and attitudes necessary to continuously improve the quality and safety of the healthcare systems in which they work. In order to accomplish this goal, six competencies were defined. These competencies from the Institute of Medicine (IOM) are patient centered care, teamwork and collaboration, evidence-based practice, quality improvement, informatics and safety. Over a decade has passed since the Institute of Medicine’s reports on the need to improve the American healthcare system. The Quality and Safety Education for Nurses
Over time the health care industry has become more complex. Health care is rapidly evolving and continuing to complicate our delivery of care, which in turn has the same effect on quality of care. This steady evolution and change results in nursing shortages and an increase in the prevalence of errors being made. In hopes of preventing these errors and creating safe and high quality patient care, with the focus on new and improved ways of thinking, The Quality and Safety Education for Nurses (QSEN) initiative was developed. The QSEN focuses on the following competencies: patient-centered care, quality improvement, safety, and teamwork and collaboration. Their initiatives work to prepare and develop the knowledge, skills, and attitudes that are necessary to make improvements in the quality and safety of health care systems (Qsen.org, 2014).
The Quality and Safety for Nurses (QSEN) project, developed in 2005 from recommendations made by the Institute of Medicine (IOM), addresses issues pertaining to how to better prepare future nurses with knowledge, skills, and attitudes (KSAs) to continue to improve the safety and quality of care provided by the healthcare organizations in which they work (Billings & Halstead, 2016; QSEN, n.d.). The mission of QSEN emphases the collaboration of all healthcare professionals focusing on education, practice, and scholarship to improve the healthcare system. Nursing quality improvement activities are essential to the financial viability of an organization and to for the safety of care delivery.
Despite many efforts to improve staffing numbers, there is often still a staff shortage among staff in hospitals and acute care settings. While the most obvious solution to short staffing is to hire more nurses, there are also other ways to make staffing more effective. A patient acuity tool is a staffing instrument that can be used to decide how much time and attention each individual patient requires. By knowing the acuity level of each patient, charge nurses can decide how many patients each nurse can be assigned to at a time. This essay describes the way a patient acuity instrument improves healthcare outcomes by promoting patient-centered care and improving on key nurse competencies including teamwork and collaboration, quality improvement, and safety measures.
Nursing is one of the most popular professions in healthcare. To become a professional nurse, one must acquire certain level of education, whether it is at associate or baccalaureate degree level. Once becoming a nurse a number of professional standards must be maintained to stay abreast with the profession. In that respect, the Institute of Medicine (IOM) in its 2010 report had suggested many recommendations focusing on the future of nursing. The Quality Improvement nursing process that I have chosen to research is patient safety. I have chosen to focus specifically on the topic of catheter associated urinary tract infections (CAUTI’s) during hospitalization and their preventions. It is estimated that 15-25% of hospitalized patients receive a urinary catheter throughout their stay, whether or not they need it. A large 80% of all patients diagnosed with a urinary tract infection (UTI) can be attributed to a catheter (Bernard, Hunter, and Moore, 2012). The bacteria may gain entry into the bladder during insertion of the catheter, during manipulation of the catheter or drainage system, around the catheter, and after removal.
According to the IOM report, the nursing profession is undergoing fundamental changes within the underlying operations of the field. A new regulatory environment coupled with increased scrutiny of the profession will dramatically impact the nursing profession. As the report indicates nurses, in the near future, will have job requirements that are business oriented. Nurses will need to have a better understanding of quality management methods in addition to overall concepts of management. As the health care system continues its rapid reform nurses must also be cognizant of the effects these changes will have on their overall roles and responsibilities. This involves a transformation of the roles and responsibilities of nurses as they enter the profession. As the health care landscape continues to change, so too must the nurses of the future. A dedication to continual learning is therefore needed to help diminish the influence of antiquated and obsolete knowledge regarding the profession.
Changes in the Health Care System and the Practice of Nursing have become complex. Technological changes, complicated client needs, short hospital length of stay, and departure from acute care to community based care, all these changes have underscore the need for professional nurses to think critically in order to provide safe and effective client care. A better educated nursing workforce can provide good health education to patients and their families. The affordable care of 2010 has required the need for nurses to expand their role of practice to meet complicated patient demands. Nothing in life remains the same things are constantly changing. In order for company to remain competitive they must make quality improvements to keep up with the changes. Health care is no different. In fact continuous quality improvement in health care are more important then any other field. Health care is a business that affects every consumer. Health care is not only a business but it is a viral part of everyone life. Because of the impact health care has on the entire world it is important that changes are made continuously to ensure that the consumer (patient) receives the highest level of service. As a competent registered nurse, my career goal is to become a healthcare quality improvement leader, a position that would enhance my commitment in promoting patient safety. I not only believe in enhancing the capacity of other care providers, but also in improving the quality of the healing environment for the benefit of both patients and their care providers. This means not only promoting collaboration with the multidisciplinary teams, but also building the necessary healing partnerships with our patients. To enhance the quality of the healing environment, I aspire to continue analyzing researches for evidence based practices and advocating for their actualization. I will continue focusing my time and energy in encouraging other nurses to improve their skills through formal education, so they can empower themselves as advocates of quality improvement for the benefit of their patients and coworkers.
