The first version of the code of ethics was named after Florence Nightingale in 1893. Designed after medicine’s Hippocratic Oath, the author, Lystra Gretter, believed that Florence Nightingale exemplified the “highest ideals of nursing” (Fowler, 2010, p. xiii) and wrote the “Florence Nightingale Pledge” in her honor. To this day, nursing schools all over the country, recite this, or similar versions at, at graduation. This public expression of nursing’s values, commitments, and duties was the foundation for the American Nurses Association’s (ANA) Code of Ethics for Nurses with Interpretive Statements (The Code) that is so well known today. The Code is a “nonnegotiable ethical standard” that “provides a succinct statement of the ethical values, obligations and duties of every individual who enters the nursing profession” ("ANA," 2016). Using The Code and the deontological decision-making model, the author of this paper will analyze the case scenario provided, and resolve the ethical dilemma.

The American Nurses Association (ANA) has the Code of Ethics which holds Nurses to the codes or provisions of these documents. I summarized Provision 1 of the ANA 's Code of Ethics. I give a scenario where this provision is broken by the nursing staff and consequences of doing so. Provision 1: Provision 1 reads as follows “The nurse, in all professional relationships, practices with compassion and respect for the inherent dignity, worth, and uniqueness of every individual, unrestricted by considerations of social or economic status, personal attributes, or the nature of health problems” (American Nurses Association 2001). Provision 1 is divided into five subdivisions. Provision 1.1 is titled “Respect for human dignity"(ANA 2001). The nurse always needs to place value on their patient as a unique individual. Provision 1.2 is titled “Relationships to patients” (ANA 2001). As a nurse you need to leave all prejudice, personal beliefs, and convictions out of the care of your patient. The patient’s self-worth and value is not defined by their religious choice, culture, lifestyle, hygiene, financial status, sex, and race. The nurse needs to form or follow an individual treatment plan that fits your patient’s personal preferences, religious beliefs, and requests. Provision 1.3 reads as follows “The nature of health problems” (ANA 2001). Nurses are constantly challenged by changes which occur in their practice environment and are under the influence of internal or external factors. Ethical issues in nursing will always be an ongoing learning process. Nurses are taught in nursing school what should be done and how. Scenarios are given on tests with one right answer. However, there are situations that nurses may encounter that may have multiple answers and it is hard to choose one. “Ethical directives are not always clearly evident and people sometimes disagree about what is right and wrong” (Butts & Rich, 2016). When an ethical decision is made by a nurse, there must be a logical justification and not just emotions.
The Code of Ethics for Nurses was created to be a guide for nurses to perform their duties in a way that is abiding with the ethical responsibilities of the nursing profession and quality in nursing care. Codes of ethics contain a coherent set of normative principles underlying a nurse’s purpose and associated values (Vanlaere and Gastmans, 2007). Two perspectives of ethics are the ethics of justice and the ethics of care (Botes, 2000). The ethics of justice constitutes an ethical perspective in terms of which ethical decisions are made on the basis of universal principles and rules, and in an impartial and verifiable manner with a view to ensuring the fair and equitable treatment of all people (Botes, 2000). The ethics of care, on the other hand, constitutes an ethical approach in terms of which involvement, harmonious relations and the needs of others play an important part in ethical decision making in each ethical situation (Botes, 2000).
To define what is the code of ethics for Nurses, we should understand that a code of ethics is a written set of principles and rules intended to serve as a guideline for determining appropriate ethical behavior for those individuals under its authority. Firstly, “The nurse, in all professional relationships, practices with compassion and respect for the inherent dignity, worth, and uniqueness of every individual, unrestricted by considerations of social or economic status, personal attributes, or the nature of health problems.” It is very important to nursing as nurses should always conduct themselves professionally and practice with respecting the rights of all persons which is not affected by race, cleanliness, illness, etc. By eliminating such influences from any personal bias or feelings the nurse has, they can prevent it from negatively affecting the care they receive. Secondly, “The nurse’s primary commitment is to the patient, whether an individual, family, group, or community.”

Nurses often encountered various ethical dilemmas in the practice setting. Both virtue ethics and caring ethics support good ethical decision making for nurses (Park, 2012, p. 149) but these are inadequate to assist in solving an ethical dilemma (Park, 2012, p. 149). For that reason an ethical decision making tool is helpful for the nurses or clinicians to come up with an ethical decision (Kelly, 2012, p. 571) that allows them to gather information, identify any gap of understanding on the issue or the disagreements between the involved parties through a clear communication (Park, 2012, p. 140). Several authors presented an ethical decision making processes (Park, 2012, p. 141), here presented the two processes that can be applied in resolving an ethical dilemma. One example of ethical decision making process is the DECIDE model by Thompson, Melia & Boyd (Allen, Chapman, Francis, & O’Connor, 2008, p. 5) and the Integrated ethical decision-making model which was derived from the combination of the different ethical decision-making models strengths (Park, 2012, p. 140). These two ethical decision-making model steps are identical to each but differ on the detailed instruction on how the steps are to be done or used in actual case.
