The Problem Of Population Health

Everyone should have the opportunity to achieve a healthy life and have comprehensive health care services available to them. To achieve this healthy life, people need to have access to the health care system and to a health care provider with whom they can develop a trusting relationship. However, existing barriers to attaining health care services often lead to disparities which in turn lead to differences in life expectancy, health status and a higher prevalence of certain chronic diseases (HealthyPeople.gov, 2012).

The health care system must change to improve our nation’s health and takes strong steps to address the unsustainable growth of health care costs in America. We still have a long way to go before our health system become effective. We still have population that do not have insurance, have difficulties accessing their health care, or their needs are not met within the healthcare system. It is an investment in prevention and wellness and increasing access to primary care physician.

Patient-Centered Medical Homes (PCMH) are growing in popularity as the right thing to do improve patient care. PCMH are growing in popularity, as there is early evidence of their effectiveness (Egge, M. 2012). The PCMH concept has been widely promoted as a way to enhance primary care and deliver better care to patients with chronic conditions. This model of care has stimulated the attention of payers, Medicaid policy makers, physicians, and patient advocates, as it has the potential to address several of the limitations of the current healthcare system (Wang, J. et al 2014). Currently, primary care in the United States is focused on acute and episodic illness, it inadvertently limits comprehensive, coordinated, preventive and chronic care (Bleser, W. et al 2014). The PCMH address these limitations through organizing patient care, emphasizing team work, and coordinating data tracking (Bleser, W. et al 2014). A PCMH and HMO have some similarities but are markedly different.

Health care spending grew 3.7 percent in 2012 and the traditional way medicine was practiced had to change (Edlin, Goldman & Leive, 2014). The Affordable Care Act and Population Health was designed based on the concept of “The Triple Aim” to foster change in patient care by providing better care for individuals, better health for populations and decrease the cost of health through improved care (Perez, 2014). As a result, population management has moved to the front by linking services, reducing hospital admission, risk stratification, pursing preventive medicine, ensuring medication review and lowering health care cost. Several organizations have follow in the pursuit of population management by forming Accountable Care Organizations

Health and human services policymakers expressed the need to move from single –disease solutions to population health interventions. The need is due to a growing public health concern- almost 1 in 3 Americans is diagnosed with multiple chronic conditions and more than 65% of health care spending is for care given to this group. Within the United States some populations groups face greater challenges then the general public with being able to access needed health care services in a timely fashion. These populations are at a greater risk for poor physical, psychological, and social health. The correct term would be underserved populations or medically disadvantaged. They are at a disadvantaged for many reason such as socioeconomic status, health, and geographic conditions. Within these groups are the racial and ethnic minorities, uninsured children, women, rural area residents, mentally ill, chronic illness and the disabled. Disparities in access to health services affect people and lead to unmet health needs, preventive medical services, and poor health. Low-income people are less able to afford the out-of-pocket costs of care, even if they have health insurance coverage. Public health insurance programs have expanded coverage for the poor and not enough to close the disparity gap. The current health care sector is too costly and too fragmented with a lot of variation in care even with established evidence based guidelines. Primary Care Physicians are constrained in their abilities to perform any proactive care that involves avoiding Hospital or ER visits, and influencing healthy lifestyles.

The population health factors is a subject of extensive discussion within American health care system, nonetheless, few comprehend the bearing to modern health care environment.

There are many issues that are causing changings in the healthcare system.
