Mr. X, a 48-year-old male was admitted with complaints of chest pain, shortness of breath, dizziness, fainting and confusion. His ECG showed that new onset of sinus bradycardia with a HR of 35-40bpm. He was scheduled for a pacemaker insertion. Upon pre-operative surgical assessment his Hb was 9 mg% and prescribed for 2 units of blood transfusion prior to the surgery. The blood transfusion started as per protocol. The initial vital signs were with in normal limits. About 45 minutes after the second unit of blood got started, the nurse observed a change in SpO2; decreased from 97% down to 94% down to 90%.
