Introduction The last week of classes for NURS1005 were a series of clinical skills activities. These activities refreshed the student’s minds on what they will be doing on placement and how to do it. We got assessed on three clinical skills; taking and recording vital signs, blood glucose levels and doing a urine analysis. I’ve chosen to reflect on taking vital signs and how I performed them. Reflection is a very important part of learning from experiences which is essential in nursing. Nursing practises continue to change and it is easier to go with and to add to that change if you are reflecting on your practise. This essentially makes nursing practises better for the patients. I have reflected on the process of how I took the vital signs during my clinical skills activities. I was very nervous but believe I performed them well due to how I was taught, what I have read and seen and the vital signs signified how my peer was acting which was healthy and within a normal range. Even though I felt I did them well, there were improvements that could and have happened since. Most of the improvements are minor in comparison to the strengths I have but it is important to recognise improvements when needed so you are giving the best and accurate care. I was aware of these improvements needed and tried to strengthen them during my clinical placement which helped me learn different ways of doing processes and also the rationale behind the processes. Throughout personal professional development reflection is essential, allowing health care professionals to self-develop by revisiting events and analysing areas in which improvements and learning would ensure a positive impact on their future practice. The Nursing and Midwifery Council, (NMC, 2010) notes that all health care practitioners must be self-aware in their own values and principles which could affect their practice. Ensuring they maintain personal and professional development while learning through supervision, feedback and reflection.
Critical reflection is vital to develop evidence based practice for safe and quality approaches to professional nursing practice. Nursing professionals should critically reflect on events to identify what health professionals might do to improve their practice and reduce the risk of a similar error. Reflective practice can help to learn from their mistakes, be empowered and most importantly to deliver best possible care to patient as nurses must work closely with their patients to develop a therapeutic relationship. Critical reflection is a valuable skill to ensure patient centred care. Assessment of a patient is a big process of decision making, it is about the collection of information which will contribute to an overall judgement of a person and the illness they may have. Lloyd (2010) states that assessment is one of the first steps which is needed to be done in the nursing process, it is a building block for a relationship and an ongoing process which lets health professionals gather the correct information to help them understand the problems and needs that the patient is going through. Most of the nursing assessment which are in use today will all have very similar aims. The difference is that how the assessment’s are carried out is where the differences come from.

Overall after reading the text InternQube: Professional Skills for the Workplace, this text was very useful as well as interesting. I was able to get a better understanding on what type of mindset I should have going into the workplace every day. This includes daily goals and routines, the way I should professional dress, how I should conduct myself on a professional level in front of people of higher status. InternQube was excellent for my first couple weeks at my internship Stenton Family Manor. First I was able to write up daily work objectives to what I needed to get done for the day. Teaching and learning in the clinical setting is not a new concept and the teaching of clinical skill to nursing student ranks high on the current agenda of nurse education (Pfeil, 2003). Therefore, has be the duty of teachers to continue to provide ongoing guidance during teaching and learning taking place. According to While (2004), the mentor is required to feel personally and professionally confident when assessing the student’s performance. This allows the development of the students will become better and more effective.
Clinical Skill Assessment Part C: Critical Reflection Description In a clinical skill assessment during week 5, I chose to perform the skill based on the case scenario number 2. I was performing under the supervision of Bianca, giving subcutaneous injection for a patient, Grace Kelly, who had undergone abdominal surgery. At the beginning, I have read the case scenario, look at the medication chart and calculate the drug before administering to the patient for 2 minutes.
The purpose of this journal is to reflect on my experience and skills gained during my clinical placement at Ben Taub Hospital. On my first clinical day, I was excited and nervous at the same time. My first placement was in the PREOP/PACU area. I was assigned to help a patient who had been in the PACU area going on 2 days. Normally, once the patient comes from surgery they are only in the PACU area for a short period of time before they are discharged home or given a bed in another area of the hospital. This particular patient still had not received an assignment for a bed. The physicians would make their rounds to come check on him daily. The patient was a 28-year-old Hispanic male, non-English speaking, he had a hemicolectomy. He had a NG tube, urinary Foley catheter, and a wound vac. My preceptor had just clocked in and she needed to check on the patient’s vitals and notes from the previous nurse. Once she introduced me to the patient and explained while I was there, she then asked me to check his vitals. (Vital signs indicate the body’s ability to regulate body temperature, maintain blood flow, and oxygenate body tissues. Vital signs are important indicators of a client’s overall health status (Hogan, 2014). I froze for a quick second. I have practiced taking vitals numerous of times and I knew I could do it correctly. I started with the temperature first, when I was quickly corrected on a major mistake I had made by my preceptor. This paper will look at the performance of blood glucose level (BGL) tests which was carried out during the clinical intensive workshops. This paper will firstly explore and discuss the value and importance of personal reflection in nursing in a health care setting and how personal reflection is a key component in the learning processes of an individual and how it improves skills. Secondly, this paper will identify blood glucose levels and reflect on how I felt I performed this skill during the clinical intensive workshop. Thirdly, discussing how I feel I preformed this skill and how constructive feedback allowed for me to improve my skill to overall result in faster and more efficiently BGL’s. Lastly, this paper will explore how I believe I can improve future BGL’s through improving my speed and how to become faster, this is through becoming more prepared prior to starting a BGL.

My interest in providing culturally competent care inspired me to become a nurse practitioner, but most of all the inspirations came from the wonderful diverse patients I have had the privilege to care for throughout my nursing career. My primary objective is to care for underserved population communities. In my career, I have seen lives been cut short that would have been saved if only they had made it to the hospital in time. But because of reasons including lack of health care access, knowledge how to access care, cultural and religious beliefs, language barrier, and fear of being judged by medical professionals led them not seek medical attention.
During my second week of clinical I had three different clinical days. It was really nice to be able to go on a Monday since that is when my nurse primarily does her wound cares. On Monday we saw 6 people and with a variety of different types of wounds. I learned the process of how to perform wound care on a patient who had a wound vacuum. This patient had a C-section which had opened twice. She also had a 4-year-old and a 17th month old in addition to her new baby. It was interesting to see much a difference the wound vacuum had made in the four days that she had it. I could see all the new tissue growth that both at the edges of the wound and its depth. This essay will discuss a clinical skill in which I have become competent in practicing. I will use a reflective model to discuss how I have achieved the necessary level of competence in my nurse training programme. The reflective model I have chosen to use is Gibbs model (Gibbs 1988). Gibbs model of reflection incorporates the following: description, feelings, evaluation, analysis, conclusion and an action plan (Gibbs 1988). The model will be applied to the essay to facilitate critical thought, relating theory to practice where the model allows. Discussion will include the knowledge underpinning practice and the evidence base for the clinical skill. A conclusion to the essay will then be given which will discuss my reflection skills, acknowledge my competence and show my personal and professional development.
As a student of nursing program I am doing 220 clinical placements at True Davidson long term care. On the first day of my clinical, I was so excited and quite nervous too. On that day, I picked one patient and it was my first experience to handle the client individually. I am going to write about that I eventually learn something from my first clinical experience. My buddy nurse told me to go to client’s room and ready her for breakfast. Then, I went her room and said her to be ready for breakfast. She looks at me and seems angry.
