On November 16, 2017, at 1236hrs, Nurse Supervisor for Desk A and B notified Police service of a sexual assault. VA Police were dispatched to Room 1F340. Upon arrival, I met with Nurse Supervisor who stated that the Veteran had been sexual assault. The Veteran was located in room 1F337, where VA police made contacted the veteran. The Veteran stated that between 0300hrs, and 0400hrs, while at a 1211 old FM 440RD Apt #1-202 Killeen TX, 76549 she was sexually assaulted be a friend. The Veteran informed the VA police that she had not showered and had changed her close. VA Police then notified Austin Police Department (APD) who had an officer respond to the clinic. At 1205hrs, APD Officer arrived on scene and attempted to get the get the Veteran to report the sexual assault to Killeen Police department. At 1337hrs, The Austin Victim Service Counselor arrived on scene and provide information to the Veteran. On 11/6/2017 at 1400 hours, I Officer Graves, responded to 735 E Haggard Ave Elon, NC 27244 in reference to an assist. Upon arrival Detective Turney, Lieutenant Sweat, and Officer Peters were at apartment K executing a search warrant. I was requested to remain outside of the residence at the front door. At 1426 hours Captain B. Tillotson entered the residence and left at 1441 hours. At hours Officer Peters released Jacy Loshin from the premises after issuing a citation. At 1455 hours Officer Chavis entered the residence and remained until the search warrant execution was completed at 1530 hours, when all remaining officers cleared.

On October 13th, 1982, at approximately 1:15 a.m. the neighbors of Queenie Volupides had reported that Queenie tore out of the house after an argument with her husband, Arthur. On September 12, 2017, members of the Metropolitan Police Department (MPD) responding to Howard University Hospital for a radio run in reference to an assault. The complainant reported to officers that Helena Lee hereinafter referred to as the defendant assaulted her by pinning her down to the ground with her knees and punching her in the face.
The reporting party (RP) stated referral #1143-3655-1102-7078895 was generated from the following referral #0534-9062-5752-307279 dated 1/28/15 regarding resident Nicole Morris age 14 DOB: 6/1/00. Per the referral Nicole is an alleged victim of sexual abuse, physical abuse and general neglect by an unknown perpetrator. According to the RP on 1/28/15 a staff member named Rebecca took Nicole to Long Beach Memorial Hospital clinic to be examined after she disclosed having suicidal ideation. Nicole revealed to hospital staff that she planned to hang herself. While waiting at the hospital Nicole disclosed being "fingered" by two different boys (names unknown) at the home. Nicole also disclosed that a girl (name not provided) beat her up. Referral 0290-1132-4581-1002176 dated 3/13/15 reported by Tatiana Garcia, Clinician with Bayfront Youth Group home.
I notified dispatch that Officer Albert and I were out at the RSC. I parked our patrol car on the north side of the RSC and entered the north door. I heard Officer Smith on the radio say he was out with the subject near the Credit Union. I observed from across the RSC Officer J. Smith and Officer R. Faison standing next to a black male all the way by the Book Store. As I walked closer, I could see the black male with both of his hands up in the air and he was getting down on both of his knees. When I got to the Credit Union, I saw a set of crutches and a black bag laying on the floor. I told Officer Albert to stay here with these items as I continued to go behind the black male. The police report ((Crash Report Number/Agency Case Number 16-238828) (Report Date 9/6/2016) has numerous inconsistencies and beginning with the license plate. The police report stated that my dad’s license plate was a “Veteran Plate” when in fact it was a “Patriot” plate depicting an American Bald Eagle, the New Mexico flag, and the U.S. flag. On 02/27/2017, I, Dillon Dickerson, badge #155, was working as a Patrol Officer for the Wichita State University Police Department (WSU PD), in Wichita, Sedgwick County, Kansas. At approximately 2300 hours during our shift briefing second shift advised we may have to do some follow up to a case they had earlier in the shift. Officer Ben Gabel #152 was the one who took the case. Officer Gabel advised there was a Battery case which happened earlier in the day. This case involved two students. Officer Gabel said there may be a male student by the name of Mekeal Simmons making contact with WSU PD. Officer Gable stated the female half of the Battery (Jessica Johnson) would be leaving campus around 0130 hours to return to Texas. Officer Gabel advised he told the female half to call WSU PD and we would come out there to make sure she was safe while she packed up her items. Officer Gable also said he was in contact with Chief Sara Morris #113 and she was aware of the case. Officer Gable said if we do have contact with the male to get a statement from him and to contact the Captains or Chief.
On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
On March 39, 2017, at 1224hrs, VA police received a Duress Alarm for 1E127. Investigation revealed that the Veteran was upset over not being able to receive her Ulfima Five Tens Unit. The MSA Clerk informed her that she would have to attend a class on how to properly us the Tens Unit. The Veteran demanded she give her the Tens Unit, because she had use it in the past. When the MSA Clerk refused and told her she would schedule her an appointment, the Veteran got angry and called the Clerk a witch and walked off. VA Police made contact with the Veteran at Primary Care Desk B lobby. Where the veteran gave an omission and stated she had an appointment and because the clerk was not at her desk she missed it. On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. On March 23, 2017 at 1402hrs, Doctor from Mental Health notified Police Service of a Health Welfare. At 1410hrs, VA police notified the Austin Police Department (APD) and spoke with a dispatcher – 7974 and requested a Health and welfare at 2407 S. 4th street (Green Doors Halfway House) for a possible suicidal Veteran. APD Dispatcher stated he had officer’s en-route to the above address and provided me the incident #170821057.
On 11/02/2015, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police, Wichita, Sedgwick County, Kansas. At approximetely 2222 hours we went to Omega Ct. to assist Sgt. Garwood since he was out on a suspicous character and another call on Omega Ct. Sgt. Garwood advised us to go to the Delta Delta Delta house at the end of the block and speak to the reporting party. I went and made contact with Julie Nolte and Dakotah Gilmore who were inside the residence. We went to a back room and closed the door.
At 1746 Hrs. Sgt. Taylor then notified me through text advising that he was “having a juvenile entered into the amber alert system”. I then arrived at the law enforcement center to assist. I met with Sgt. Carroll and asked him to begin assisting after his briefing. CID Sgt. M. Sheedy was then notified of the incident and responded along with Det. Lee. A-Chief was also at the law enforcement center and was aware of the situation.
On February 7, 2017 at 1538hrs, I was dispatched to the information desk for possible weapons in a vehicle. Upon arrival I met with Veteran Raymond Blanchette - 2671. He had come in to facility to ask about his firearms that were in his motorhome. Blanchette state he lost his house and was living out of his RV. Further RV was his way to get back and forth to appointment. I decided to warn him about the weapons being in his RV. I based this decision on the fact he, had to use walker to get around, being Homeless and being an elderly man. I did inform him if brought the RV with weapons back on station we would charge him with a weapons violation.
