Concept Introduction: This is an important creative tool to identify possible causes associated with a failure/problem (effect of the causes). This is a group activity and includes brain storming also if required.

Interpretation: Select the top two actions to be taken immediately.
Concept Introduction: The number of problems are many, but the resources are limited. Moreover any action must be such as to provide a solution effectively.
In such cases, the problems are evaluated based on previous available data. The problem showing the highest frequency, obviously will give the maximum respite from the problem.
